Design Nationwide population-based cohort study Methods Between 20, 1,384,427 pregnant women were registered in the National Health Insurance Research Database in Taiwan. The purpose of this study is to explore long-term stroke risks, differentiating subtypes and their time trends. Objective No study ever investigated the long-term risk of stroke in women with pre-eclampsia/eclampsia. The recombinant standards are potent calibrators, highly stable, easy and inexpensive to produce, and overcome the limitation associated with preparations derived from pooled human serum, expediting the production of next generation standards with different specificities to successfully tackle β-lactam allergy delabeling by in vitro tests. Conclusions: The new diagnostic strategy compares favourably with the results obtained on the ImmunoCAP system, paving the way towards the standardization of penicillin allergy testing. The sensitivity of the in vitro test significantly increased (66 %), doubling that of the ImmunoCAP reference in vitro assay with an overall specificity of 100 %. Results: Testing a cohort of 65 human serum samples, the method achieved a good agreement and strong positive relationship, reaching a limit of detection below 0.1 IU/mL. The novel standard targets the major antigenic determinant of penicillin G and the paratope of Omalizumab, acting as human-like specific IgE. Methods: Serum samples from a cohort of allergic patients and controls were analysed by a chemiluminescence-based immunoassay, using a bispecific binanobody as standard. We aimed to tackle a novel diagnostic strategy for specific IgE testing based on a homologous interpolation scheme, using recombinantly produced standards. However, discrepant results associated to the low sensitivity of the in vitro tests have limited their routine diagnostic use for delabeling purposes. Instead, the quantification of specific IgE is highly useful to confirm immediate allergy to penicillins. In addition, it improves our understanding of how opioids alter spinal pathways involved in the sensations of pain and itch.īackground: Penicillin allergy delabeling initiatives are now part of antibiotic stewardship programs and include the use of invasive and risky in vivo tests. This study selectively and systematically characterises the synaptic connections between RVM projection neurons and their SDH targets to advance our knowledge of how this descending projection is organised and modulated. Conclusion and Implications: Developing reliable and effective alternatives to opioid analgesics requires a detailed, mechanistic understanding of how opioids interact with nociceptive circuits. By contrast, the mu-opioid receptor agonist DAMGO inhibited only a subset of RVM-SDH synapses, acting both pre- and postsynaptically, while the delta-opioid receptor agonist deltorphin II had little effect. This inhibitory synapse was presynaptically suppressed by the kappa opioid receptor agonist U69593. These SDH neurons had diverse morphological and electrical properties. Key Results: We demonstrate that SDH neurons are targeted by descending GABA/glycine inhibitory inputs from the RVM, although glycinergic inputs predominate.
Experimental Approach: We used ex vivo optogenetic stimulation of RVM descending fibres and whole-cell patch-clamp recordings from superficial dorsal horn (SDH) neurons to identify the target neurons and to investigate their descending synaptic inputs. However, our knowledge about the spinal synaptic targets of RVM projections and their modulation by opioids is unknown. In addition, this projection plays an important role in mediating the analgesic effects of opioids. Background and Purpose: Descending projections from neurons in the rostral ventromedial medulla (RVM) make synapses within the superficial dorsal horn of the spinal cord that are involved in acute nociception and the development of chronic pain and itch.
